Patients helped to stay at home with housing associations new scheme
Peaks and Plains Housing Trust have launched a rapid response falls services which helps keep patients out of hospital and reduced ambulance call outs.
Peaks and Plains are working with the North West Ambulance Service to create a TrustLink service which receives referrals from patients who have previously called for an ambulance after a fall, reports 24dash.
TrustLink will send a specialist ‘Falls Advisor’ to visit a patient at home in order to assess their risks and give advice to prevent future falls. Following that the advisor will then work with a range of other agencies to offer support services.
Launched in November 2014 to combat winter pressure, and part funded by NHS Eastern Cheshire CCG Resilience Fund, the falls project has successfully forged partnerships with health, social care and the third sector.
Paul Walton, Pathfinder area manager for Cheshire and Mersey, North West Ambulance Service NHS Trust, said: “Ambulance crews can confidently refer patients, who after a clinical assessment are deemed appropriate for referral to the scheme, with the knowledge that the patient will be seen quickly by a team of experienced and caring healthcare professionals, from within their own community. The team are able to identify reasons or issues causing the patient to fall, review their medication and primarily support the patient to live safely in their own home.”
